Patient-Centered Medical Homes Reduce Costs [CFAH..org]

The patient-centered medical home (PCMH) is a model of health care that emphasizes personal relationships, team delivery of care, coordination across specialties and care settings, quality and safety improvement, and open access.

Medicare expenditures for patients who received care from PCMHs grew more slowly than for Medicare patients who did not.

Rates of emergency room visits and costs for acute care hospitalizations seemed to be lower for patients receiving care in PCMHs than other patients.
