Camden Coalition receives support from state to continue Housing First program and its work as a Regional Health Hub [camdenhealth.org]

From Camden Coalition of Healthcare Providers, July 8, 2021

The Camden Coalition of Healthcare Providers will receive over $1M in funding to support its Housing First initiative and its work as a Regional Health Hub in South Jersey. This funding comes as a result of the 2022 fiscal year budget signed by New Jersey governor Phil Murphy on June 21, 2021.

Launched in 2015, the Camden Coalition’s Housing First initiative provides safe, dignified housing to frequently hospitalized and housing-unstable individuals in Camden, and connects them with extensive support services so that they can improve their health and manage chronic conditions. The model is funded in part by the New Jersey Department of Community Affairs, whose support will now continue as a result of Governor Murphy’s recently passed budget.

“We are so pleased that our work providing stable housing to individuals with complex health and social needs can continue thanks to the support of both the Governor and the Legislature,” said Camden Coalition CEO Kathleen Noonan. “Through Housing First, we have seen how secure housing can dramatically improve the health and well-being of our participants. It is incredibly gratifying to see the state address such an important social determinant of health in its quest to improve the lives of all New Jerseyans.”

In addition to support for Housing First, the Camden Coalition will also receive funding from the New Jersey Department of Human Services to continue its work as a Regional Health Hub in Southern New Jersey. As such, the Camden Coalition supports the State of New Jersey’s health priorities by providing healthcare data infrastructure and analysis, supporting care management, and convening community stakeholders in close coordination with the Office of Medicaid Innovation. The organization also serves as a local expert and conduit for state health priorities, convenes multi-sector partners in South Jersey to take action on the state’s most urgent health needs, and operates a regional Health Information Exchange to ensure that health and other data are accessible across community organizations with the goal of ensuring continuity of care.

The Camden Coalition is one of four Regional Health Hubs created as a result of legislation signed in 2020, along with Healthy Greater Newark ACO, the Trenton Health Team, and the Health Coalition of Passaic County.
