One module will feature perspectives on addressing Social Determinants of Health during the pandemic, reducing health disparities, and developing and nurturing community partnerships to ensure services are delivered to those who need it most, when they need it most. This module of the event will kick off with an exciting panel discussion entitled Evaluating the Impact of Social Determinants of Health on COVID-19 Outcomes: Coordinating Efforts with Community Programs and Resources to Address Inequalities and Disparities. Lauren Flynn Kelly of AIS Health will lead the interactive discussion, which will highlight insights from Genesee Health Plan, CareSource, Health People: Community Preventative Health Institute (a CBO perspective), Count My Health and Unite Us.

This module will also feature case studies from leading Medicaid organizations who will share how:

  • United Healthcare Community Plan Louisiana and Mom's Meals launched a rapid food response to the pandemic for the most vulnerable populations and leveraged policy flexibility to fund and implement a home-delivered meals benefit
  • Health Net addressed implicit racial bias as a systems intervention to improve access to care and health outcomes
  • Mississippi State Department of Health integrated data sources that measure SDoH to effectively treat the whole person
  • Health New England leveraged Community Health Workers during COVID-19, utilizing virtual and in-person workers as agents of change
