The Institute on Trauma and Trauma-Informed Care has a trauma-informed organizational evaluation tool that looks at trauma-informed culture/climate, policies/procedures, readiness to change, etc. that's been adapted for a number of systems and departments.

You can see a reduced version of the culture/climate scale via our Trauma-Informed Organizational Change Manual, and our evaluation team would be happy to set-up a time to discuss what could make sense based on what you are hoping for and their experience evaluating trauma-informed change.
