Hi Elizabeth! Great questions and I love how you are framing this around the experience of the person being served. Often this can be overlooked in our day to day work, so I am very pleased to read your question.

I wonder if the attached SAMHSA Treatment Improvement Protocol #57 (image below) may offer some answers to the issues you raised? And the SAMHSA website has great information and resources that may be useful too.
