Original ACE measure

Hi All,

I'm in search of the original ACE questionnaire used in the original 1998 study (Felitti, V.J., et al., Relationship of childhood abuse and household dysfunction to many of the leading causes of death in adults. The Adverse Childhood Experiences (ACE) Study. Am J Prev Med, 1998. 14(4): p. 245-58.)

There seems to be differing versions floating around and the link in the original article that directs readers to the questionnaire used doesn't actually take me to the original questionnaire.
