Health Share of Oregon has been working with several Portland area pediatric clinics to develop foster care medical home models. The CCO is currently working with Randall Children's Pediatric Clinic, Hillsboro Pediatrics, and OHSU Doernbecher General Pediatrics to evaluate and evolve their models. Happy to discuss further.
