I know of a project in WA State that is somewhat similar, it's grant funded and the project itself is winding down, but one component is implementation of a trauma screening tool with children and youth in foster care, you may find some helpful information:

Creating Connections Project

This resource out of San Diego also talks about a lot of the same tools being used in the WA project, specifically the trauma screen - SCARED.

It looks like both projects focus on the same outcome - increasing better connections to MH and EBP services for children and youth in foster care.
