Gathering and using local data

I am interested in finding how other local/regional communities might begin to gather/use local ACES data.  I have usually found local data more compelling in changing local institutions that reference to national studies.  Could ACEs plus some sort of protective/resilience score be adapted into an evidence -based approach for screening and prioritizing children for preschool programs for at-risk children?  I imagine that gathering that information for preschool aged kids is a challenge with a lot of threats to validity.  Also, I have seen some statewide approaches to gather routine ACES data about their population.  Are there similar efforts in use that scale to the local regional level (regional populations of around 250,000)?  When/how is it most appropriate to collect local/regional ACES data that could be compared to some of the national studies and how is that local data translated into action.  A lot of questions I know.  Thanks for any leads you could provide.
