Asking parents about kids' adversities - any models to recommend?

We are looking at models of collecting information via a telephone survey  with parents or guardians regarding their child's exposure to adversities and possibly also resilience and would appreciate any comments or experience that people have regarding this, especially related to these three items:
  • When asking parents/ caregivers about what adversities their children face, can anyone share how they have done this?
  • Are there known assessments for reliability and validity of ACE reporting using parents as proxy for their children?
  • How have others addressed age-related issues and differential reliability/validity of responses for ACEs when using parents as proxies of children at different developmental stages (i.e., infants and toddlers through older adolescents)?
