However, looking for high quality practices in unexpected places – with people and in communities whose limited access to such resources necessities human resourcefulness – reminds us to dive deeper and discover what really “counts” about the human capacity to care. A family-centered community health system (FCCHS) is the organization of people, institutions, and resources in a specific geographic area that deliver health care and other health-influencing services. In an FCCHS, services are delivered in new ways that build partnerships between providers and families; that are organized in a coordinated network; that work to dismantle historical and structural racism; that promote community-wide supports for foundational relationships and early relational health; and that make visible high-performing medical home and team based care practices; all of which are effectively coordinated with community supports and networks. This webinar will discuss the elements of the FCCHS that when adopted and coordinated, can drive transformation at the system, community, and policy levels and bring forward commentaries from thought leaders and advocates.
