Round Valley Indian Health Center - Mendocino County
Sacramento Nativev American Health Center - Sacramento County
Sonoma County Indian Health Project - Sonoma County
Tule River Tribe - Tulare County
United American Indian Involvement - Los Angeles County

This grant supports:
*Infrastructure support
*Renovations/Remodels
*Build-Outs
*Equipment and Furniture

The MAT Access Points Project is funded through the Department of Health Care Services’ California MAT. Expansion Project and is administered and managed by The Center at Sierra Health Foundation.
