Building Community Capacity to Address ACEs: Community of Practice

July 25, 2017 from 9:00am-12:00pm.

Sonoma County ACEs Connection and partners brings you together to raise awareness about Adverse Childhood Experiences and resiliency. This Community of Practice Session will help each participant to continuously improve their knowledge and ability to share ACEs science with fidelity, as well as to realize important core values in the work. 

ACEs and Resiliency Fellowship Participants, including Master Trainers and Presenters are invited to attend this session. Master Trainers will be registered by our staff as their participation is required. Presenters will register by purchasing a Presenter Program Participant ticket.

We also invite up to 35 community members to join us.  Please purchase a community member ticket.

Presenter Learning Objectives: 

1) Understand key features of the Self-Healing Community Model and share the impressive results and strategies the State of Washington has had in implementation of this model.

2) Discover the significant overlap of frameworks and explore opportunities for greater impact through connecting existing networks and coalitions in Sonoma County.

3) Explore how the Self-Healing Communities Model can be used to “connect the dots” across existing networks and build community capacity as a pathway to a healthy, just and resilient community.

Presenter: Ellen Bauer, PhD

Ellen Bauer is Public Health Director for the Sonoma County Department of Health Services.  Prior to this assignment, she was the founding manager of Health Action, a coalition of diverse community leaders convened by the Health Department to identify priority health issues in Sonoma County and to offer leadership to implement initiatives and policies to create a healthier community  

Before coming to Sonoma County, Dr. Bauer had over 20 years of experience designing and implementing national and community-level health program evaluations.  She has led research and evaluation on a wide range of health issues, including sales of sodas in schools, asthma, long-term care services and insurance, and implementation and outcomes (quality, utilization and cost) of managed Medicaid for both acute and long-term care services. 

She earned a BA in Economics from Stanford, a Masters of Public Policy, Graduate School of Public Policy, University of California at Berkeley, and a Ph.D., Health Services and Policy Analysis, University of California at Berkeley.  She was also awarded several pre- and post-doctoral fellowships from the National Institute on Aging and the Agency for Health Care Policy and Research.

Presenter: Oscar Chavez

Oscar Chavez is the Assistant Director of Human Services for the County of Sonoma. Prior to joining the county, Oscar served as Executive Director of the Community Action Partnership of Sonoma County, one of the largest community benefit organizations in Northern California. Prior, he was Director of Business Support Services and Director of Engagement and Public Relations for a large community-based organization in Bakersfield, CA.

In his current capacity Oscar oversees the County’s Upstream Investments Initiative; a policy framework designed to strengthen our community’s ability to implement evidence informed practices and shift funding toward prevention. He is also responsible for the Department’s planning, research and evaluation functions, as well as community and client engagement.

Oscar serves as: board chair, Latino Service Providers; chair of Sonoma County Community Development Commission; member of Sonoma County Health Action Council; board member, Community Foundation Sonoma County; member, California Stewardship Network, a statewide effort to promote regional collaboration and stewardship; steering committee member, Rise Together, a nine-county Bay Area initiative committed to ending poverty.

In 2016, Chavez was honored by Hispanics in Philanthropy, a network of nonprofits, corporations, foundations and individuals dedicated to addressing poverty issues, identifying solutions and mobilizing Latino communities in the U.S. and Latin America.

 In 2012, he received the Sonoma County Jefferson Award and the North Bay Business Journal non-profit leadership award. In 2008, he was named one of the North Bay’s “40 Under 40” leaders by the North Bay Business Journal.  

Oscar lives in Windsor with his wife Christine and their four boys.

This program is pending approval by the California Board of Registered Nursing, Provider #CEP 12787, Contact Hours XX of continuing education credits, awarded by the County of Sonoma, Department of Health Services.  

Course meets the qualifications for 3 hours of continuing education credit for LMFTs, LCSWs as required by the California Board of Behavioral Sciences.

The Sonoma County Behavioral Health Division is approved by the California Association of Marriage and Family Therapists to sponsor continuing education for LMFTs, LCSWs, & LPCCs. Sonoma County Behavioral Health Division maintains responsibility for this program/course and its content.  Provider Number 135030, Exp. 02/06/2018

This course has been approved by California Board of Registered Nursing, B.R.N Provider No. 10298 expires 5/31/2018.
