Dr. Andres Sciolla has shared this unique opportunity that dovetails with one of the conversations that emerged in our meeting discussion yesterday- how to best serve "high utilizer" medical patients, whose recurring presence in emergency rooms is often related to their social needs, which in turn are impacted by ACEs and trauma.

The conference will focus on how we can deliver better care through coordinated, data-driven, and patient-centered approaches—with attention to the social correlates of health like housing, transportation, hunger, and education. It's an opportunity to connect, learn, and share, as we bring together pioneers and new practitioners working around the country, creating together a national hub that will unite and advance a new field of health care."
