Please join us in October to take an in-depth look at the Collective Medical Platform! We are grateful to our Managed Care Organizations (MCOs) for sponsoring this valuable resource for our partnering providers to use. We are also glad that many of you are already experiencing the benefits Collective Medical has to offer.
Among these benefits include notification of patient visits to an Emergency Department or hospitalizations, enabling many of you to close communication gaps that have historically occurred across care settings.
At the October Learning Collaborative, Collective Medical will share the functionality of the platform and explain how to contact them when there are questions or concerns. We have also invited our partnering providers to present their use of Collective Medical. Please join us! |

The Learning Collaboratives are designed to assist healthcare provider organizations participating in the GCACH Practice Transformation program. GCACH uses the Greater Columbia Cares Model (GCCM) to embed evidence-based practices that lead to better health, better care and lower costs. Registration is free and open to the public.
