AHRQ topic nomination and future for federal guidelines

TIHCER meeting. June 22, 2019.

Thank you so much to Ellen Goldstein and Martina Jelley for allowing a brief presentation to the Oklahoma based meeting today re: the ACES TIC AHRQ topic nomination by the American College of Preventive Medicine ACPM work group.

The dialog and slides from today were excellent (and I have shared some already with others). A notable quote was “all of our patients are traumatized”. Another take away is that supportive care for the effects of trauma “can reduce ED visits and costs by one third”. This is consistent with what Vince Felitti MD found at Kaiser Permanente in SoCal. He has data from a cohort of 430,000 patients that demonstrated a one third reduction in ambulatory care visits by pre visit screening followed by the clinician asking “do you believe that your early trauma experiences have affected you in later life.”?

The SAMSHA guidance can indeed influence other federal policy makers. Here they are again. https://store.samhsa.gov/product/SAMHSA-s-Concept-of-Trauma-and-Guidance-for-a-Trauma-Informed-Approach/SMA14-4884.html. I did share these again today with the AHRQ topic nomination science group.

The USPSTF discussion is important for the future of federal healthcare policy, though it may be a decade away as an evidence base on outcomes is developed.

The quote from the IPV USPSTF statement applies to effective TIC intervention. Support must be ongoing. Quoting “provide or refer women who screen positive to ongoing support services.”  https://www.uspreventiveservic...le-adults-screening1

This USPSTF IPV Position statement can be a template for us to the future aspiration to have USPSTF examine trauma informed care and make recommendations.

It is our hope at ACPM.org that AHRQ.gov will move forward with the ACES TIC topic nomination.  We stand ready at ACPM to produce a position statement in the next year with a goal of publication in the American Journal of Preventive Medicine. Please share slides, research, papers  and data that may assist us. My contact information is Kevin Sherin MD, MPH. Sherinkmj@gmail.com.
