The Kern County Medically Vulnerable Care Coordination Project was formed in 2008 and has over 50 partner organizations. Our Vision – Use coordinated services to measurably improve outcomes for Kern County infants and children, prenatal through 5 years of age, who are at risk of costly, lifelong medical and developmental issues.
Among the greatest challenges faced by our collaborative case management partners are the long-term effects of childhood trauma on Kern County mothers, fathers and their children as parents and providers work to improve birth outcomes for their at-risk infants and children in Kern. Based on the results of multiple meetings since early 2016, attracting over 250 participants, MVCCP partners have committed for 2017 – 2020 to expand their care coordination system level efforts to facilitate a Trauma Informed Care model among parents, providers and all public and private partner organizations that will lead Kern to become a Trauma Informed County over the next few years
Our November 2, 2017 conference was MVCCP's 7th Annual such gathering and brought together, for one day, 140 participants from multiple agencies and community organizations at Cal State Bakersfield to learn about ACEs, the long term effects of trauma, and how to implement Trauma Informed Care Coordination, all in advancing the next phase for Kern to become a Trauma Informed County.
