ACEs Aware Network of Care Implementation grant totaling $3,000,000.00

The Sacramento County Office of Education (SCOE) received a grant award notice today for the Adverse Childhood Experiences (ACEs) Aware Network of Care Implementation grant totaling $3,000,000.00 for the contract period of February 1, 2021 – June 30, 2022. This successful proposal is a result of your commitment and support for this work and your role in this community partnership.

ACEs Aware Implementation grant funds will launch the Sacramento County ACEs Aware Trauma Informed Network of Care Collaborative, a cross-sector network encompassing representation from all six sectors: healthcare, public health, social services, early childhood, education, and justice. The Roadmap for Resilience (RFR) will be used to guide and inform this work. This comprehensive plan will enhance the connection of Medi-Cal and other health care providers to community resources that incorporate primary, secondary, and tertiary prevention strategies.



Goals  

  • increased referrals for families to appropriate preventative supports
  • increased access to trauma-informed training in Sacramento County
  • increased or enhanced diverse resources and supports to prevent or mitigate toxic stress for families, especially those in Sacramento County’s most vulnerable communities

Sacramento County Office of Education (SCOE) will serve as the lead agency and River City Medical Group (RCMG) will serve as co-lead, as the medical lead in Sacramento County.



Partners

ACES Aware builds upon partnerships and activities that are already in place in Sacramento County. This grant leverages and incorporates these opportunities to provide a strong foundation for this Network.

  •      Foster Youth and Homeless Services
  •      Special Education
  •      School-Based Mental Health and Wellness
